    Key Duties may include (but are not limited to):

  • Connecting and linking people into the community with local activities and recreational interests
  • Supporting people with daily living skills such as shopping, cooking, travel, training
  • Performing tasks in relation to domestic assistance (e.g. mopping, vacuuming, gardening, feeding pets)
  • Providing meal assistance and meal preparation
  • Medication prompting assistance and monitoring
  • Assisting with showering, dressing and personal care related tasks with the use of manual handling aids if appropriate
  • Basic tasks that may include booking appointments, assisting with shopping, paying bills, liaising with service coordinator
  • Accompany to social group and other activities if required

    Minimum requirements:

  • Rights to work within Australia
  • Working with Children Check clearance
  • Qualifications in Certificate 3 Aged Care, Certificate 4 in Disability /HACC / Individual Support
  • Current First Aid Certificate and CPR
  • Current Victorian Drivers licence
  • Police check, dated within last 3 months
  • At least three months experience working in Aged Care, Disability, Home and Community Care is preferred
  • Social Welfare, Community Services and Mental Health Certificate 4 qualifications are advantageous

    Key Duties may include (but are not limited to):

  • The NDIS Administrator is responsible for the effective coordination and completion of administrative requirements in relation to recording and claiming for service delivery under the NDIS.
  • Manage NDIS client packages through the provider website and portal
  • Establish and monitor funding packages through our Client Management System
  • Review ongoing funding and follow up with stakeholders
  • Verify data and issue invoice batches for payments
  • Follow up issues and concerns with the NDIS office
  • Training and support of staff on system as required

  • Police check, dated within last 3 months
  • Ability to use computer software and social media
  • Good communication skills, both oral and written

    Key Duties may include (but are not limited to):

  • Responsible for financial sustainability of Disability Services, ensuring budgeting requirements are met, viability tested and keeping up to date with financial changes issued by the NDIS
  • Commitment to continuous improvement principles, research, education and adherence to the set quality standards
  • Staff Management of Team leaders including performance management and mentoring
  • Responsible for obtaining feedback from clients and staff, particularly their views on current service delivery and creating business development opportunities by increasing stakeholder engagement
  • Responsible for implementation of systems and processes that place the client at the centre of service delivery, exercising maximum choice and control including NDIS, Quality and Safe Guarding standards
  • Programs are a balance dignity of risk vs duty of care requirements for the organisation and are inclusive of meeting ratio and rostering requirements
  • Ensure that all staff understand clients' human rights, support them to ensure their rights are upheld
  • Responsible for ensuring staff members receive ongoing training in disability matters

    The primary objectives of the Disability Service Manager include:

  • developing staff skills in the delivery of a person-centred approach, as well as ensuring compliance within the NDIS Practice Standards
  • The role ensures that GNCS delivers services that meet individual goals of the client, enabling increased independence and identity in the community as well as being proactive in researching disability trends and advancements to better improve our services
  • The role has face-to-face time with clients, families and employees, as well as strategic requirements such as ensuring that quality standards are met
  • This role is responsible for resolving and responding to organisational/ client needs both strategically and immediately as they occur
  • The role will build rapport with clients and families through excellent customer service

    The Support Coordinator is responsible for the following:

  • Working with customers to achieve their goals in line with their NDIS plan
  • Providing ongoing support coordination services to customers in line with their NDIS plan
  • Using a coaching approach to assist customers in capacity building
  • Maintaining accurate and up to date case notes on customers progress
  • Ensuring Work Health and Safety guidelines are adhered to
  • Engaging with customers, their supports and 3rd party organisations in a professional manner
  • Undertaking work in line with the organisations policies, procedures and values
  • Complying with all relevant legislation and codes of conduct
  • Maintaining an up to date knowledge of the NDIS systems and practices
  • Maintaining clear professional boundaries with customers
  • Maintaining an up to date knowledge of local services and organisations customers can utilise

  • Monitoring and Reporting: Regularly monitoring the health status of participants and reporting any changes to the appropriate professionals or family members.
  • Education and Support: Educating participants and their families about health management, and providing support to empower them in making informed decisions about their care.
